What a Real Executive Physical Looks Like — And Why Yours Probably Fell Short

Most high-achieving professionals treat their annual physical as a checkbox. Fifteen minutes with a physician, a CBC and metabolic panel, blood pressure, weight. Maybe a lipid panel. You're told your numbers look "within normal limits" and sent on your way for another year.
The problem is not that this exam is wrong. The problem is that it is designed to detect disease after it has already established itself — not to identify the biological trajectory that leads to disease a decade before it becomes symptomatic.
For executives and high-performers who understand that their health is their most leveraged asset, there is a meaningful gap between the care they're receiving and the care that is actually possible.
What Standard Labs Do Not Measure
A conventional annual physical typically orders a complete blood count, a basic or comprehensive metabolic panel, a lipid panel, and perhaps a TSH. Together, these panels might assess 20 to 30 individual markers. They are designed to rule out overt disease states — uncontrolled diabetes, severe anemia, flagrant thyroid dysfunction.
What they do not capture includes biological age versus chronological age, early cardiovascular risk beyond total cholesterol (ApoB, Lp(a), oxidized LDL, and inflammatory markers like hs-CRP), insulin resistance in its subclinical phase before a diabetes diagnosis, mitochondrial function and cellular energy production, heavy metal and toxin burden, detailed hormonal health across multiple axes, gut microbiome composition and intestinal permeability, early cancer signals through liquid biopsy or GRAIL technology, cognitive resilience markers, and genetic risk variants that meaningfully affect longevity strategy.
This is not an exhaustive list. It illustrates the scale of information that is clinically available but routinely not ordered — often because conventional medicine has not built the infrastructure to act on it, and because insurance structures do not incentivize preventive depth.
What 700+ Biomarkers Actually Reveals

The cardiovascular module goes well beyond a standard lipid panel. It includes ApoB, the most accurate measure of atherogenic particle number; Lp(a), a genetically determined risk factor that is present in roughly 20% of the population and dramatically elevates cardiovascular risk, yet is almost never checked in a standard physical; hs-CRP and other inflammatory markers; and advanced imaging options for patients where early structural assessment is indicated.
The metabolic module includes a detailed insulin resistance assessment — fasting insulin, HOMA-IR, and glucose variability data — because insulin resistance is one of the most consequential and reversible drivers of accelerated aging, and it is frequently present for years before blood sugar elevates into a diagnosable range.
The hormonal assessment is comprehensive across all relevant axes: testosterone and its metabolites, estrogen and progesterone where applicable, DHEA-S, cortisol mapping across the day, detailed thyroid evaluation including T3, reverse T3, and thyroid antibodies, and growth hormone markers.
The biological age assessment includes epigenetic methylation testing — what I use in practice as TruAge — which measures how quickly your cells are aging relative to your chronological age. This is one of the most actionable longevity metrics available, because biological age is not fixed. It responds to lifestyle interventions, targeted supplementation, hormonal optimization, and stress reduction in ways that are measurable and trackable over time.
Early cancer detection is integrated through GRAIL Galleri liquid biopsy when appropriate, which screens for over 50 cancer types through a single blood draw, detecting signals that can appear years before a tumor would be identifiable through standard imaging.
